Filters That Reduce ‘brain Clutter’ Identified

ScienceDaily (Apr. 19, 2011) — Until now, it has been assumed that people with conditions like ADHD, Tourette syndrome, obsessive compulsive disorder and schizophrenia -- all of whom characteristically report symptoms of "brain clutter" -- may suffer from anomalies in the brain's prefrontal cortex.

Damage to this brain region is often associated with failure to focus on relevant things, loss of inhibitions, impulsivity and various kinds of inappropriate behaviour. So far, exactly what makes the prefrontal cortex so essential to these aspects of behaviour has remained elusive, hampering attempts to develop tools for diagnosing and treating these patients.

But new research by Julio Martinez-Trujillo, a professor in McGill University's Department of Physiology and Canada Research Chair in Visual Neuroscience, has brought new hope to these patients. He believes the key to the "brain clutter" and impulsivity shown by individuals with dysfunctional prefrontal cortices lies in a malfunction of a specific type of brain cell. Martinez-Trujilo and his team have identified neurons in the dorsolateral sub-region of the primate prefrontal cortex that selectively filter out important from unimportant visual information. The key to the normal functioning of these "filter neurons" is their ability to, in the presence of visual clutter, selectively and strongly inhibit the unimportant information, giving the rest of the brain access to what is relevant.

"Contrary to common beliefs, the brain has a limited processing capacity. It can only effectively process about one per cent of the visual information that it takes in," Martinez-Trujilo said. "This means that the neurons responsible for perceiving objects and programming actions must constantly compete with one another to access the important information.

"What we found when we looked at the behaviour of the neurons in the prefrontal cortex, was that an animal's ability to successfully accomplish a single action in the presence of visual clutter, was dictated by how well these units suppressed distracting information."

These results could be highly relevant for identifying the causes and improving the diagnosis and treatments of a wide range of mental disorders including ADHD and schizophrenia.

The research was conducted by Therese Lennert, a PhD student who holds a Vanier Scholarship, and it was funded by the Canada Research Chair program, Canadian Institutes of Health Research (CIHR), EJLB Foundation, and Natural Sciences and Engineering Research Council of Canada (NSERC).

More Accurate Diagnosis of Alzheimer’s

ScienceDaily (Apr. 19, 2011) — A new study from the University of Gothenburg, Sweden, shows how analysing spinal fluid can help to detect Alzheimer's disease at an early stage. The researchers behind the study hope that their findings will contribute to a greater international breakthrough for this type of diagnostic method.

It all comes down to biomarkers, substances that are found at abnormally high or low levels in patients who go on to develop Alzheimer's. The most common biomarkers to be identified by the researchers in the spinal fluid of patients with Alzheimer's are proteins and peptides -- short chains of amino acids.

"What's new about our study is that the biomarkers are really good, better than in the past, as the study was carried out extremely carefully with suitable participants via clinical trials and well implemented and controlled laboratory analyses," says docent Johan Svensson, who is working with professor Kaj Blennow's research group at the Sahlgrenska Academy, which has long been involved in researching the development of these biomarkers and advocating their use.

A total of 60 patients who were being investigated for dementia took part in the study, along with 20 healthy controls.

"We measured levels of the biomarkers in the spinal fluid and found that high levels of these substances confirmed the diagnosis of Alzheimer's with a high degree of accuracy compared with levels in healthy controls and patients with other forms of dementia," says Svensson.

"We also saw that patients who hadn't yet met all the clinical criteria for Alzheimer's had similar levels of the biomarkers in their spinal fluid to patients who had developed the disease fully."

The research group therefore concludes that these measurements can also be used to identify Alzheimer's during the early stages of the disease. In such cases, the biomarkers can be used to identify those patients with mild symptoms who are most likely to benefit from treatment.

"If a medication that affects the course of the disease does become available, it will probably be most effective during the early stages, and these biomarkers could be used in the development of such a medication," says Svensson.

The study will be published in the Journal of Alzheimer's Disease.

Dois grupos brasileiros trabalham na vacina contra a dengue

Três pesquisas acenam com a possibilidade de criação de vacinas contra os quatro tipos de dengue no Brasil, que poderão ser usadas no programa de imunização do Ministério da Saúde.
O desenvolvimento das vacinas segue critérios internacionais de homologação de produtos farmacêuticos para uso humano, com três fases de testes de segurança e eficácia comprovada em amostras consecutivas.
A pesquisa mais adiantada é a do laboratório francês Sanofi Pasteur, que está na segunda fase de estudos clínicos. Os testes ocorrem no Brasil, Peru, na Colômbia, em Honduras, no México, em Porto Rico, Cingapura, nas Filipinas, na Tailândia e no Vietnã.
Em todo mundo, mais de 4 mil pessoas receberam uma ou mais doses da vacina.
No Brasil, o estudo clínico é feito desde agosto do ano passado pelo Núcleo de Doenças Infectocontagiosas da Universidade Federal do Espírito Santo, com um grupo de 150 crianças e adolescentes de 9 a 16 anos.
A vacina em teste é ministrada em três doses e o estudo tem a duração de 18 meses. A perspectiva da comunidade científica é que a vacina esteja disponível daqui a cinco anos, após registro na Agência de Vigilância Sanitária (Anvisa).
Vacina brasileira contra a dengue
O Instituto de Tecnologia em Imunobiológicos Bio-Manguinhos, ligado à Fundação Oswaldo Cruz (Fiocruz), no Rio de Janeiro; e o Instituto Butantan, vinculado à Secretaria de Estado da Saúde de São Paulo, também fazem pesquisas para criação de uma vacina contra a dengue. Se conseguirem desenvolver as vacinas, as patentes serão nacionais.
De acordo com o diretor médico de ensaios clínicos do Instituto Butantan, Alexander Roberto Precioso, a primeira fase de estudos clínicos terá início no segundo semestre, com pessoas adultas que não sofram de diabetes, nem de hipertensão e que não tenham problemas nos pulmões ou no coração, bem como nunca tenham tido dengue, nem febre amarela.
"O objetivo principal é demonstrar a segurança da vacina", afirma Precioso. De acordo com ele, a vacina foi testada em macacos, nos Estados Unidos. A patente da vacina testada foi cedida com exclusividade para o Butantan pelos Institutos Nacionais de Saúde (National Institutes of Health - NIH).
Já em Bio-Manguinhos, duas vacinas estão sendo desenvolvidas. A primeira experiência, exclusiva, é uma vacina recombinante feita a partir do vírus da febre amarela e do vírus da dengue. A outra tentativa, mais adiantada, é feita em parceria com o laboratório belga GlaxoSmithKline. Essa vacina está em estágio pré-clínico, de testes em animais.
Segundo a bióloga Helena Caride, que gerencia os projetos de desenvolvimento tecnológico das vacinas em Bio-Manguinhos, a intenção é fazer uma vacina que só exija duas aplicações em intervalo mais curto do que o da vacina em teste pela Sanofi Pasteur.
Prevenção
Helena lembra que o desenvolvimento de vacinas contra a dengue no Brasil não permite descuidar do combate ao mosquito Aedes aegypti, transmissor do vírus.
"Ainda temos um caminho bastante longo para ter essa vacina no mercado e não sabemos como essa vacina vai se comportar. É muito importante continuar batalhando no combate ao mosquito vetor", ressalta Helena, ao lembrar que "só a ciência de ponta não consegue fazer sozinha.
O saneamento básico é fundamental. Questões de cultura e educação no Brasil também são fundamentais para melhorar a saúde pública."

Cirurgia pode curar câncer reincidente de próstata

Tratamentos para o câncer de próstata
Uma pesquisa feita nos Estados Unidos, na Europa e no Brasil mostrou que o câncer recorrente de próstata pode ser curado.
Até agora, a doença, quando aparecia pela segunda vez, após o tratamento com radioterapia, era considera incurável.
Os pacientes com o diagnóstico de tumor maligno na próstata têm duas opções de tratamento: a cirurgia para a retirada da glândula - que pode trazer complicações, como a impotência, em 50% dos casos, e a incontinência urinária - ou o tratamento com radioterapia, que também pode deixar sequelas, mas em menores proporções.
É possível curar
Nos casos em que a opção pela radioterapia, após o tratamento, o tumor voltava, a doença era considerada sem cura, e o único tratamento disponível é o da hormonoterapia, que consiste em bloquear a produção do hormônio masculino no organismo.
Isso retarda o desenvolvimento do câncer, mas não o cura, e dá sobrevida, em média, de mais dois anos ao paciente.
De acordo com um dos coordenadores da pesquisa, o médico do Instituto do Câncer de São Paulo, Daher Chade, a operação de retirada da próstata, depois da reincidência do câncer, não era feita em razão de, geralmente, a radioterapia produzir aderências na região da glândula, o que prejudicava a cirurgia.
"O que foi descoberto com essa pesquisa é que nessa situação, os pacientes que fizeram radioterapia, e o tumor voltou, a cirurgia pode ser feita com segurança. Com a radioterapia mais moderna, a radiação ocorre mais no interior da próstata para eliminar o tumor, e causa menos aderência ao redor da glândula", afirmou.
Sobrevida surpreendente
O estudo publicado na semana passada, e que começou a ser feito nos Estados Unidos há cerca de 25 anos, apresentou resultados surpreendentes para os pesquisadores.
Cerca de 77% dos pacientes que fizeram a cirurgia após o reaparecimento do tumor estavam, dez anos após a cirurgia, sem nenhum sinal de disseminação da doença.
Outro dado importante obtido, foi a constatação de que 83% dos pacientes estavam vivos uma década após a cirurgia.
"Esse é um tempo muito longo para um tumor que se achava incurável, e que era instituído um tratamento de hormônio, para que o paciente vivesse só mais dois ou três anos".
O médico ainda destacou que os médicos não precisam aprender uma técnica nova para fazer cirurgia, já que o procedimento é o mesmo já utilizado nos casos em que o tumor aparece pela primeira vez.
"Os urologistas já sabem fazer essa cirurgia. O que é preciso é adquirir experiência, porque, até agora, poucos cirurgiões faziam essa cirurgia porque não se sabia que ela era benéfica para o paciente".
A pesquisa analisou 404 pacientes de todo mundo, que tinham, em média, 65 anos de idade.
